American/European Channels access from Malaysia ?

Does anyone know a satellite installer who knows how to set up the satellite to receive channels from the US or Europe ? is there any way to get a decoder or a card for this purpose ?
Any help on this would be great.
Also, does anyone receive channels from the west ?

The only European channels provider you can receive is Multichoice whereby cover most american european programme. The free to air channels cover most european non english programme.
